Nursing visits will consist of a complete physical assessment and evaluation of the patient condition. Assessments are done prior to commencement of your case. The initial assessment allows us to gain a full understanding of the services that you require. The Skilled nurse will also instruct the patient and family/caregiver in disease process, management and medication. You will also be given the opportunity to ask any questions regarding your care.
HOME HEALTH AIDES
Under the guidance of the skilled nurse, Home Health Aides will provide appropriate assistance with activities of daily living such as personal care and ambulation.
MSW (MEDICAL SOCIAL WORKER)
The Medical Social Worker will provide information and support with financial, emotional and social aspects of your health and well being. They also assist with community resources and long term care planning.
PHYSICAL/OCCUPATIONAL/SPEECH THERAPIST
This team of skilled professionals will provide therapeutic procedures necessary to assist patients with mobility, exercises and communication techniques.
